If you are registered to attend any of the OC FOOLS H.O.T. classes at the convention, you MUST bring this signed waiver to participate in the class. For those who have registered, you should have received this form via email. If you did not, or if you have registered within the past few days, please print the form below, have it signed, and bring it with you to the training.

**Students will not be allowed to participate in the class if they do not have this signed form.
